An amusement device, fee schedule, and other requirements are defined by the Commonwealth of Massachusetts General Laws Title 20 Chapter 140 Section 177A.
This is an annual permit that is issued under the authority of the Town Manager and is valid from January 1 to December 31st of a calendar year. The following items must be received in the Town Manager's office in order to apply for this permit; only complete applications will be processed:
Fee (to be submitted with the application): $100 Per Device
Application Materials
- Cover letter explaining the request (including specific information about the number and type of device(s), plus any significant details that are not provided in the application form)
- General Permit Application
- Amusement Device List Affidavit
- Tax Certification Statement
- Workers' Compensation Insurance Affidavit
- A check for the appropriate fee based on the rate made payable to: Town of Westborough
